To get started, please download and complete our Rider Application Packet and Liability Release forms below. 

 

Please note that parents/caregivers are required to be present at lessons and assist as needed, and also must complete the Volunteer Application Packet & Liability Release forms.

Once forms are completed please return them to CPR by:

Once we've received all the forms, we'll give you a call to set up an initial lesson.
